Schools require the following documents to register:

• Proof of child's birth date - Documents can include: birth certificate, passport, visa, legal documentation, previously verified school records, etc. 

• Proof of residency - Required two documents listed in tab 1: "Establish residency". 

• Records of Immunizations - All students enrolling will need a completed Certificate of Immunization (CIS) form through the Washington State Department of Health. Please gather proof of immunization from your family physician or local health department, then transfer this information to a certificate of immunization to sign and verify accuracy. • Consent for Exchange of Information (if applicable) - If a student is transferring from another school, the parent or guardian much give their consent to transfer records from the student's previous school. Please have the name, address, and phone number of the previous school ready prior to registering.

• Transcript or Academic History (if applicable) - Students transferring from another school should bring academic records.
