The event continues Riverview’s ongoing partnership with Costco Pharmacy
Beginning next week, Riverview School District will once again welcome Costco Pharmacy, to provide an optional, on-site vaccination event for families wishing to access the COVID-19 Pfizer vaccine for their children. To be eligible to receive a vaccine at this particular clinic, children must be between the ages of 5 and 11 on Nov. 9, 2021 (or Nov. 20 for first dose on that day).

First dose appointments:
Tuesday, November 9th, 8:00 a.m. - 12:00 p.m.
Saturday, November 20th, 8:00 a.m. - 12:00 p.m.
Second dose appointments: The second dose appointment will be automatically scheduled after receiving the first dose.
Tuesday, November 30th, 8:00 a.m. - 12:00 p.m.
Saturday, December 11th, 8:00 a.m. - 12:00 p.m.

Important things to know the day of the vaccine clinic:
All vaccine recipients must have a consent form completed by a parent or guardian. Please download, print, and fill out the consent form provided below. If you are unable to print the form, paper copies will be available at the clinic. Note: Consent forms are not submitted or saved online.

The vaccine is available at no cost to you. While insurance is not required, if you do have insurance please bring your card with you on the day of the clinic.
One parent or guardian must be present at the clinic with their child(ren). Only one parent/guardian will be allowed to accompany each child inside the clinic.
To lower the risk of the spread of COVID-19, upon arrival, we will take your temperature and ask you questions about COVID-19 symptoms, exposures, and travel.
Masks are required at all times inside the ESC.
